Purpose:
The purpose for completing incident reports are to insure a record is
maintained, to notify ISC of the circumstances surrounding a possible insurance
claim or should any lawsuit result, or if an injury results in extensive future
medical treatment and, most importantly, to evaluate prevention strategies to
ensure the future safety of DeMolay youth and Advisors. To assist Oregon DeMolay Advisors, there are two incident report forms: (1) Injury and (2) Property. You may download these forms, fill in the information and email the completed reports to the Risk Management Advisor, who will review the reports, edit and forward them to the Executive Officer for final approval before they are sent to ISC. If you have any questions, please call the Risk Management Advisor or email them. Injury: If you are an Advisor at
the scene of a very serious injury, the first concern is lifesaving.
Immediately insure notification of emergency medical personnel by
contacting 911. Advise dispatchers
of the nature and extent of the injury. They
may often be able to guide you through some first aid measures until Emergency
Medical Technicians arrive. If the injury does not
require an immediate ambulance response, administer first aid and consider
obtaining in-field assistance from EMTs or transporting the victim to a hospital
or clinic for medical treatment. As soon as possible, notify
the victim’s parent. Your Chapter
Advisor should be carrying the emergency medical notification and consent forms
with information concerning parental consent, specific medical needs and
allergies and personal insurance information. Make a note of the
approximate time the injury occurred and the time the parent was notified.
Obtain names of witnesses, emergency personnel, the ambulance company,
the attending doctor or nurse. The
address/location of the incident should be noted and the actions or behaviors,
which lead up to the incident: (i.e. James was playing broomball when he
suddenly slipped on the ice, backed into Paul, fell to the ice and struck his
elbow.) You will need to describe
the injury. You also should ask the
hospital or clinic an approximate cost for the treatment, and the ambulance fees
as well. Property:
Property
reports are intended for incidents in which there are no injuries, but property
damage or a theft has occurred. (For
example, during a Degree a piece of a Lodge’s furniture is accidentally
damaged, or during a dance a DeMolay’s wallet is stolen, or a DeMolay is
involved in a traffic accident during a sanctioned Chapter event.)
In cases involving theft,
consider assisting the victim to call police.
Many homeowners’ insurance will not cover a loss unless a police report
is filed. If an officer responds, be
sure to obtain their name for the Incident report. Also include a description of the lost, damaged or
stolen property and an approximate value, for example: during a football game the ball struck the passenger side
window, breaking it. Approximate
cost to replace: $200. |
